The media definitely has an agenda and are definitely trying to politicize this pandemic, as evidenced by the latest "Breaking News" Tweet by WRAL.

Here is the comment I posted back to their Tweet:

Great response.

I know comments are dangerous to read....but reading them about any Covid statistic completely destroys any thoughts of "this should not be political".
You can absolutely see political leanings in basically every single response...well, at least in the "angry" responses. 4 cases....to your point way below the norm given the # of people tested, and yet people are acting like it was 1000 people having a keg party and dancing on tables in a room for 10 and that now the world is coming to an end.

Funny thing with comments on Twitter aimed at my comment (I assume there may be some). I do not read any of the comments to any Tweet I post. So, people are wasting their time on Twitter if they are commenting back to me. I assume most of the negative comments are from people too ignorant for me to try and persuade, so I am not going to waste my time with them.

Why anyone associated with NC State pays attention to them is beyond me.

They are essentially a hostile entity to the university. They are directly across the street and we should be their "home town team" but are not and never have been.

I bet Gravely can tell some stories about how he had to frame certain things when he was there.

At some point in the near future, the State should reach a point of diminishing returns regarding back-filling data.

I don't think the "story" is about how many tested positive. The story is the White House holding a political event with 1,500 people packed together without masks in the midst of a 100-year pandemic and, guess what, some of those people were COVID positive. The percent positive is irrelevant.

Well at least you are being consistent... now how about those rioters confronting people (in their personal space) on their way out of the event? No comment about them I'm sure... of course not!

False positives could be an issue in some cases. Something we need to be mindful of when our cases do decrease.

Was the story about the event at the White House? I thought it was about what happened in NC.

Today's report contains over 870 cases from over 2 weeks ago. Worst backlog report EVER.

Worst data dump by the state yet. Between mid-July and today, the highest percentage of cases outside of 9 days was 18% on 7/28. And only 3 times were more than 10% of reported cases more than 9 days before the reporting date.

Today you had a full 33% from PRIOR 14 DAYS

Just to show, despite the media panic. The old case reporting has no effect on our CURRENT trend... the data almost not relevant.

Just raises valley slightly.

Why?
Presenting numbers in a negative manner without the context that Davie provided. The actual fact reads a lot different than the way it was presented.
I don't think the "story" is about how many tested positive. The story is the White House holding a political event with 1,500 people packed together without masks in the midst of a 100-year pandemic and, guess what, some of those people were COVID positive. The percent positive is irrelevant.
Was the story about the event at the White House? I thought it was about what happened in NC.
Yep, sure is. I don't know what the **** I was looking at, or thought I was looking at. This is why I need to spend more than ten seconds on a thread at a time!

Yes, the secondary peak is primarily due to colleges.

The shift of the date of specimen collection trend line was due to 1000 cases that LabCorp didnlt report.

Was reading the CDC site and came into this article. Apparently Coronavirus is the only cause of death posted in just 6% of the people who are listed as dead by coronavirus. Everyone else died due to something else and Coronavirus was listed as a secondary cause...and we have shut down the entire country for 5 months over this. It's time to open everything back up, send the kids back to school, and let get football season started

That's not the good news you think it is. That means on 6 percent the actual thing that caused the death BECAUSE of covid wasn't listed. Covid should almost never be the only COD. Look at the table. The COD is actually ARDS, pneumonia, respiratory arrest, cardiac ischemia, etc SECONDARY TO covid. Covid caused the condition that finally led to death. Does that make sense? That 6 percent number means nada except the percentage of death certificates incompletely done for one reason or another.

I'm confused on this. So you're saying Covid caused the comorbidities? Maybe I'm misinterpreting but I took it to mean patients presented with Covid along with other underlying conditions (obesity, COPD, kidney disease, etc) and that only 6% died from Covid with no other underlying conditions.

I, too, work in a hospital and we've not had a single death from Covid where the patient wasn't already "circling the drain", as the doctors often say, prior to getting Covid, However, the death (in all cases) was listed as Covid.

Here is a very interesting article from the New York Times on testing and the issues of using the PCR testing results to make policy decisions. The article suggests what is needed is the use of more rapid tests which is something we have on the horizon. In fact, the federal government just signaled they intend to purchased 150 million rapid tests just for this purpose.

Read the whole article here: Your Coronavirus Test is Positive. Maybe it Shouldn't Be.

Some key points from the article, if you do not want to read the whole thing:

1. Patients identified as positive may carry "insignificant" amounts of virus.
"Some of the nation's leading public health experts are raising a new concern in the endless debate over coronavirus testing in the United States: The standard tests are diagnosing huge numbers of people who may be carrying relatively insignificant amounts of the virus."

2. Number of cycles required to identify the virus is important
"The PCR test amplifies genetic matter from the virus in cycles; the fewer cycles required, the greater the amount of virus, or viral load, in the sample. The greater the viral load, the more likely the patient is to be contagious."

"This number of amplification cycles needed to find the virus, called the cycle threshold, is never included in the results sent to doctors and coronavirus patients, although it could tell them how infectious the patients are."

"In three sets of testing data that include cycle thresholds, compiled by officials in Massachusetts, New York and Nevada, up to 90 percent of people testing positive carried barely any virus, a review by The Times found."

3. Amplification cycles as high and greater than 40 are too high for identifying contagious cases.
"Any test with a cycle threshold above 35 is too sensitive, agreed Juliet Morrison, a virologist at the University of California, Riverside. "I'm shocked that people would think that 40 could represent a positive," she said."

"A more reasonable cutoff would be 30 to 35, she added. Dr. Mina said he would set the figure at 30, or even less."

4. CDC says it is difficult to find viable virus above a threshold of 33 cycles. North Carolina's lab's threshold is higher than 33, making the test overly sensitive.
"The C.D.C.'s own calculations suggest that it is extremely difficult to detect any live virus in a sample above a threshold of 33 cycles."

"North Carolina's state lab uses the Thermo Fisher coronavirus test, which automatically classifies results based on a cutoff of 37 cycles." <== The difference between 33 and 37 is not insignificant, since amplification is exponential

5. In one recent analysis, limiting the cycles to 35 would cause half of the positive samples to no longer be classified as positive.
"With a cutoff of 35, about half of those tests would no longer qualify as positive. About 70 percent would no longer be judged positive if the cycles were limited to 30."

6. When shown results of one study, some doctors are shocked at the information and believe we need to change our thinking about testing.
" 'I'm really shocked that it could be that high the proportion of people with high C.T. value results,' said Dr. Ashish Jha, director of the Harvard Global Health Institute. 'Boy, does it really change the way we need to be thinking about testing.' "

"The number of people with positive results who aren't infectious is particularly concerning, said Scott Becker, executive director of the Association of Public Health Laboratories. 'That worries me a lot, just because it's so high,' he said, adding that the organization intended to meet with Dr. Mina to discuss the issue."

7. Rapid Testing could and likely is more important as a diagnostic tool
"The F.D.A. noted that people may have a low viral load when they are newly infected. A test with less sensitivity would miss these infections."

"But that problem is easily solved, Dr. Mina said: 'Test them again, six hours later or 15 hours later or whatever,' he said. A rapid test would find these patients quickly, even if it were less sensitive, because their viral loads would quickly rise."

"But for the outbreaks raging now, he said, what's needed are coronavirus tests that are fast, cheap and abundant enough to frequently test everyone who needs it even if the tests are less sensitive."

I'm confused as well. Shouldn't that mean unless there is an underlying condition then we should get back to normal?

Yes, I think you're misinterpreting. Yes, people with comorbid conditions like obesity, DM, and HTN are more likely to die if infected, but those conditions in and of themselves aren't a cause of death. People don't show up in the hospital and die of being fat. They die when their comorbid conditions lead to inability to avoid complications of things like covid.

People don't die of covid. They die from complications of covid. The asymptomatic and those who don't die of covid are the ones who don't develop a cause of death like those listed for the other 94 percent of the people in the table listed - ARDS, respiratory arrest, pneumonia, myocardial events, etc which are direct complications of covid. They didn't just come in circling the drain from ARDS and then just die and by happenstance test positive for covid of which they were completely asymptomatic. The covid CAUSED the ARDS, and then rightfully the cause of death is then ARDS SECONDARY TO COVID. It's written out that way correctly on death certificates and thus only 6 percent have covid as the only cause of death.

If there a small percentage of relatively sick people who are infected with sars-cv19 but never develop the disease (covid), ie are completely asymptomatic, and show up in the ER and die of a heart attack unrelated to the infection? Sure. Likely happens occasionally. But how do we know the infection didn't cause the heart attack? Based on the pathophysiology of this disease and it's way of affecting the heart and causing micro clots, it probably DID. But to try to explain away all these deaths as people who were "dying anyway" and just happened to have an symptomatic Cv19 infection is really far fetched.

Unless there have been very few deaths where you work I have a very difficult time believing your last paragraph for the reasons I explain above. Unless you and I are there treating docs and filling out their DCs we'll never know, but I think it's very unlikely that Cv19 is so prevalent and yet so asymptomatic in such sick individuals that are already "circling the drain" from some random unassociated etiology that their deaths are completely unrelated to covid.

Lastly, I've tried to explain this before. Covid, even if determined to NOT be the IMMEDIATE COD, will rightfully be listed for surveillance reasons. Especially during a pandemic. This is NOT done to make the numbers look bad to ruin your life for some political reason. Some aholes may try to use the data for that I'm sure. But it's done for community health reasons so the people charged with making public health decisions have some real clue as to how this disease is affecting our communities from a public health perspective. If you want to be mad at politicians for using such data for political gain I don't blame you. But being angry it's listed that way is not useful because it's being done so correctly.
