Unlike the mRNA vaccines, the Novavax vaccine is a protein-based vaccine. It contains a fully-formed spike protein (unlike the mRNA vaccines) and it is this protein that causes the body to stimulate a response. It does not contain any genetic material, so the concerns raised by many about the vaccine potentially altering ones DNA (a very spurious claim) are not applicable. The vaccine is more similar to influenza vaccines, but contains no live or dead viruses. It is impossible for it to infect you with Covid-19.

Based on the vaccines design, I (and others) believe it will be more effective in treating multiple variants of the virus - more so than the current mRNA vaccines. In published trial results, it was 90% effective in preventing severe illness and hospitalizations for all known variants (including Delta) at the time of the trials. I have not seen results for its efficacy against Omicron, but I believe it could be equally effective against it as well.

Since it is using a more traditional method of vaccine development, it should be more acceptable to people who have hesitation about mRNA vaccines. It also has the potential of decreasing allergic reactions experienced by some people, since it does not contain polyethylene glycol (used in mRNA vaccines). Polyethylene glycol is the primary reason why some people are unable to receive an mRNA vaccine.

Given these reasons and given the fact it will stimulate my body in a different way than my last vaccine dose, I have chosen to wait on this particular vaccine - assuming it gets approved and distributed for use in the next couple of months.

He's in the thick of patient care every day and engaging with others that are also. He goes to great lengths on here to dispense sober, reserved, unbiased commentary about the state of hospitalization data, therapeautics, vaccines, and other treatments through the lens of a practicing physician.

Correct. The J&J vaccine does not contain mRNA. It is known as an adenovirus vaccine. A adenovirus vaccine is a vaccine that includes a virus that has been modified to include a small genetic portion from the SARS-CoV-2 virus (the portion that codes the spike protein). Adenovirus' have been modified in way that does not make you sick, can not replicate, and can not impact your DNA. Once it is injected, it goes into your cell and your body will make the spike protein. The spike protein then comes to the surface of the cell, where the immune system will recognize it as an intruder and attack it.

The Novavax vaccine contains a fully formed protein (the spike protein) that matches the spike protein from the SARS-CoV-2 virus. It is manufactured by inserting the spike gene into another virus and this virus is allowed to infect moth cells. This infected moth cells stimulate the production of additional spike proteins that join together to form spikes. These spikes are then harvested and they are used in the vaccine. The vaccine also includes another chemical that helps to amplify the reaction of the body to the spike protein.

So, in the case of the J&J vaccine, it relies on your body to make the spike protein. In the case of the Novavax vaccine, the spike protein is injected into the body fully-formed. It does not rely on the body's cells to manufacture the spike protein first.

In probably something that only interests me (and again, the data is subject to backlog). There is still an interesting pivot point in Deaths. Again, I expect deaths to increase just due to the sheer number of cases (and both WITH/FOR). People are going to die and when possibly as high as 10% of the population is ACTIVELY infected then people are going to die with COVID.

While this is just correlation and doesn't mean anything, the peaks of both our major waves of 'Deaths, Date of' eventually came in just under 2 weeks from our case peak. The week of 12/18 is when Omicron really started taking hold... look at the current death curve two weeks later:



Again, I full expect this to bounce in the near term just due to volume. But even looking at death by report date, on Jan 12, 2021 we were reporting a 7 day average of 92 deaths a day... and today our 7 day average is 32.

Until things change (and a lot of backlog comes in), my hypothesis is that is the Omicron effect on what was our winter wave which up to then was primarily Delta.

At least here, I'm hearing of very few deaths from COVID-19, and a critical care guy told me last week, "they're not making it to us." Meaning the admissions aren't sick enough in general for the icu anymore. It is so prevalent in our communities that it is stressing the system, and yes some will die with covid, but it's very different this time around. Met with an ER friend yesterday, and he said he's "never seen it like this in my 25 yr career." It's not that the ERs are overrun with covid per se, but unlike the alpha wave in which every other medical issue seemingly shut down (MIs, strokes, bleeds, falls, etc) and allowed us to focus soley on covid, this time everything else is much busier PLUS the CLIs are coming in in large numbers. But we all think the peak is near just as Wayland and others have graphed... We think within 2 wks conservatively.

I think the 36 RBD mutations of the cron has essentially defanged the virus as we have always felt a novel virus could eventually do, and while there will still be those who suffer more than others, it will eventually be what most would consider another endemic cold or flu variant. Cases will be milder, outcomes will be better, and hysteria will finally fade.
