Examples include Election of 1876, Tweed Ring in NYC and Daley Machine in Chicago to name a few. Of course, I'm sure that most elections overall have been fair. But, to think that the fraud can't occur and not to be vigilant to ensure that it doesn't happen would also be a mistake.
