I would think this is more about population density than anything. Large cities have higher vaccination rates but the people living there come into close contact with more people on a daily basis.

Hasn't it also been established that the vaccines have less protection against becoming infected with Delta and basically no protection from Omicron?

I think a more accurate data set would be number of infections in vaxxed per 100k vs unvaxxed per 100k in each city separately.

I found out yesterday that I have three more friends who are fully vaccinated plus boosted who tested positive two days ago. Two are feeling okay with minor cold symptoms and one has had body aches and chills for two days. This is now a total of seven close friends who are fully vaccinated AND boosted who have become infected with the virus.

Given what I have seen from my close circle of friends is this:

1. Vaccination and boosting will NOT keep you from becoming infected, now that the Omicron variant is making its rounds. As I have posted before, the Omicron variant is able to evade the antibodies better than any of the past variants - no matter if you are boosted or not.
2. Your T-Cells and B-Cells, acquired from the vaccines, or past infection, will limit your symptoms and severity of the virus in most cases. This is the reason to get vaccinated - not to prevent infection, but to reduce severity, hospitalizations, and deaths.

We are at a point now where a lot more people will become infected and those who are vaccinated, for the most part, should have positive outcomes and recover quickly. Those who are not vaccinated or who have not been previously infected should be concerned if you fall into the high risk categories (over-weight, pre-existing conditions, compromised immune system, older age group, etc). With Omicron, I believe we are at the point where Covid is endemic and everyone will become infected at some point in the future, just like they do with the common cold, flu, and other circulating diseases.

The idea we can control rates of infection through the use of controls (masks mandates, testing, boosters) is a fallacy. The virus is now too transmissible. We can control severity of infection and recovery with the vaccines and the treatments we have for people who do become infected.

Visited the folks in NC this week and listened to a couple of podcasts from Rogan during the drive back to Austin yesterday. The interview with Dr Robert Malone was very informative. He has personally been canceled by Twitter because he posted comments encouraging full disclosure of vaccine risks despite being one of the most important vaccine development Drs in the world. This interview is episode #1757 on the Joe Rogan Experience podcast. Highly recommended.

"IF" Omicron replaces Delta (which as of mid-December we still had likely a majority Delta cases but the rapid acceleration in the back half of December and anecdotal evidence suggest Omicron is spreading fast), the wild-type vaccine may be OBE.

Not arguing that current vaccine doesn't impact potential severity of illness to Delta and prior, but I would think we would want to tailor the vaccine to a new dominant circulating variant.

Potentially, given the trajectory of Omicron worldwide, you may be largely SOL if you chose to get the vaccine now, and unfortunately have to deal with the more severe Delta consequences if unlucky enough to get it (since you still have to wait a few weeks for vaccine to be effective)

I mean, it is certainly possible that Delta kicks around for even longer, given that it was in a seasonal growth phase when Omicron showed up (as opposed to SA which had low spread when Omicron showed so it was able to rapidly outpace Delta to the tune of 99% cases in SA are Omicron where very early in their seasonal wave they still had as much as 30% Delta).

<shrug> Just spitballing. It is certainly possible that Delta could be around longer, in which case, may not be a bad idea. Guess we will see in the next few weeks how the competition goes.

Update locally:

We're at 192 admitted. ER and ICUs completely full, ER with covid (50 in holding awaiting admission), ICU not with covid. I personally opened up 2 beds today in case they were needed, but i bet they'll go to trauma pts. LOS for new admits is now 2-3 days and very rarely needing icu, so we think we're mostly getting omicron. Symptoms have changed. Much more short duration fever, emesis, diarrhea. Don't have omicron vs delta data. We have to get that from the state or cdc, and we think they're 2 wks behind. Used to be 90% of the admitted were unvaxxed, now closer to 75%. I think percent positive is about 15%.

Outpatient tx: a really busy day during the previous spikes was about 90-100 referrals/day to the covid outpt clinic for consideration of covid-specific treatment. Wednesday and Thursday topped 500 pts/ day. Unfortunately, we're only given enough sotrovimab to treat 7 pts/day. Remdesivir has been opened back up for outpt use, and we will receive enough oral anti-virals to treat a few pts/day. In all we'll be able to treat about 30 pts/day with outpt therapy. There are protocols in place to help determine who qualifies (the most) for the available doses. (The doc i met with today has treated 1000s of covid pts inpatient and outpatient and has yet to see a case of renal failure with remdesivir).

Seems like about a doubling since the last update before Christmas?

Lots of gastro symptoms? I hadn't heard those talked a lot with Omicron.

Yes. The majority are still uri sxs (most commonly the sniffles, mild sore throat), but certainly seeing plenty of puking. Diarrhea has always been part of the disease, but omicron seems to have a plenty with the syndrome of a GI bug... Fever, puking, diarrhea for a half day followed by lack of appetite and night sweats

It appears the Omicron wave is starting its downward trajectory in London. Just like we saw in South Africa, cases went up quickly with an R=2+ and then fell very rapidly after peaking. In London, the R value is now less than 1 and cases reported daily are really starting to fall rapidly. I expect to see a similar trend in the US and in particular, in NC, with a peak in the next 10-14 days.

It now appears it is okay to say cloth masks do very little to slow down infections for SARS-CoV-2. This is inline with a study I posted months ago regarding mask effectiveness in preventing infection from an airborne virus. In summary, cloth masks are not very effective in stopping an airborne virus. The most effective are "fitted" N95 masks and they far exceed anything offered by cloth masks.

I think we have reached a point where the vaccines have to be updated to reflect the latest variants. Omicron is showing a strong ability of evading the antibodies produced by the current vaccines. The T-cell and B-cell response to the virus is still very good and this is why vaccination is still an important tool to limit severity.

Someone should update that ridiculous meme from early on about peeing on someone if you're wearing pants that people tried to use to show that any face covering worked. There's a big difference if one of the people is wearing a wetsuit and the other is wearing fishnets. How many people stood and close talked each other under a false sense of security because of a cloth mask?

I get it, they didn't want to overwhelm the supply of masks for healthcare workers, and I get that they thought it probably didn't hurt. But to tell reasonable, intelligent people that wearing a t-shirt sleeve over your face provided the same protection as an N95, and to call those people all kinds of names for pushing back against that was just flat wrong. Then, instead of just admitting that the protection wasn't the same, the people in charge tried to convince the public, with a straight face, that they should wear two or three masks, and people actually did it! It feels like something out of a sketch comedy skit.

I'm glad it's starting to not be taboo to acknowledge basic common sense, but I'm convinced that for the rest of our lives there will be people walking around believing in some of this stuff.

The problem with your Wake Med graphic is it doesn't show age stratification. Here we are, two years in, and people like you refuse to understand that it doesn't take a phD to look at simple statistics and make a risk calculation. If those you worship would live in the realm of facts and figures based on real life numbers instead of fear mongering, threats and wishful thinking, there wouldn't be a need to convince those who need to be vaccinated to do it.

It isn't actually meant to get anyone vaccinated.

It serves a couple of purposes, 1) To say to their vaxxed political base they are the Good PeopleTM and their political opponents are the Bad PeopleTM 2) Create scapegoat/outgroup to blame when their policies inevitably fail.

China is following a "zero Covid" strategy. For example, if you fly to China and plan to stay for longer than a few days, you have to quarantine in a hotel room for 21 days and you are tested daily. A couple of weeks ago, I was speaking with a co-worker in China and they were shocked that we are able to go to places without a requirement to wear a mask. They can get in serious trouble with the government if they try to do something in public without wearing a mask.

What China is currently experiencing is a vaccine that was not very effective to begin with and is now doing much worse against the latest variant.

Hospitalizations while going up, are still below what they were elsewhere. I have seen reports out of other hospital systems that incidental admissions could be in excess over 50% (I have seen numbers as high as 80% in UK).



For whatever reason, my county charts keep getting blocked by IPS. But Wake County is drunk on testing.... also might be time to rethink the CDC Thresholds ... or just remove them, as they mean nothing.
